I've been a medical marijuana patient for a number of years and now I am to renew but i want an extended plant count which basically means I can buy 1/2 - 1 pound of weed at one time. Laws where I live have just changed to where that has become harder and the doctors writing the cards need to have you bring medical records to show them in order to get a higher buying limit. Does anyone know where I can get an editable medical history form? Or any advice at all on the matter? I may even qualify but I don't know who to contact to get the correct records but I'm not 100% sure that would qualify me so I'd rather just create one myself if at all possible to avoid being rejected.
